Please look over this form (or print it out) before beginning to fill it out, and note any information you need to look up. Look it up, and then come back and fill out the form, because this form does not allow you to save some answers and then come back later and do the rest.

Also, after you fill in a box, don't hit Return or Enter to go to the next box. On some browsers hitting Return or Enter submits your survey.  Rather, use Tab or Point-and-Click.  When you are done, click Submit at the bottom. 

Type your answers directly into the boxes. If you cut and paste from a full-featured word processor like Word, which uses nonprinting characters, submission may fail at the final submit step. If you really want to cut and paste, do so from a pure text program like NotePad or TextEdit.

Attendance Options

     4 weeks

Note: There is no 2-week option this year, at least not yet. We prefer to fill the camp with 4-week campers. If you cannot commit to 4 weeks if accepted, do not apply at this time. Check back in the spring, to see if we have enlarged the camp to make room for some 2-week campers. If you can commit to 4 weeks but strongly prefer 2 weeks, explain why in the final comment box below. The 2-week option was created for younger students who have not been away to sleepover camp or who are not sure they are ready for all-day math for 4 weeks.

Our experience is that your recommendation was still submitted, but it was truncated where the error occurred &ndash probably a non-ascii character you inserted, perhaps by cut and paste.
